Created personalised SVI score which determines the vulnerability of patients using the social determinants of health conditions of patients. Patient’s demographic and social history data was used as SDOH variables. Used Pandas library for structuring the available data. The aim was to correlate the index with the clinical outcomes of the health of a patient.
