The aim of the sub-project "I&C solution" is to contribute to the development of processes and procedures to reduce the number of transfers of dying persons in care settings that are not medically and nursingly indicated and to improve the quality of care in the last phase of life: target-oriented transfer of adequate information to the right actors or participants at the right time. --- Das Ziel des Teilprojekts "IuK-Lösung" ist die informationstechnische Mitwirkung bei der Entwicklung von Prozessen und Verfahren zur Reduktion von medizinisch-pflegerisch nicht indizierten Verlegungen Sterbender in Versorgungssettings und die damit verbundene Verbesserung der Versorgungsqualität in der letzten Lebensphase: zielführende Übermittelung adäquater Informationen an die richtigen Akteure bzw. Beteiligten zur richtigen Zeit.
